Sports Physicals, Camp Physicals, and College Physicals
We do sports physcials for most programs in Gwinnett County!
As kids return to school or go off to summer camps, we offer quick and reliable physical exams.
At Gwinnett Urgent Care, we provide sports physicals, camp physicals, and several other standardized physicals for children for $45. College physicals are offered for $100.
The state of Georgia requires that all students have a standard sports physical examination before participating in school athletics. Many other camps and area athletic programs have similar requirements. Will your kids be ready to participate?
Bring in your required forms or download the Georgia Pre-Participation Evaluation Form. Filling out the required portions before you come in saves time!
If you are under the age of 18, also bring in our Authorization to Treat a Minor form or have a parent/guardian with you.
